A simple, practical framework for supporting prenatal bonding and early attachment awareness — built for real conversations and everyday care.
Pregnancy BASICS is a reflective, strengths-based assessment tool designed to help professionals observe and support bonding between parents and their unborn baby throughout pregnancy.
The tool was developed in direct response to a key service gap: no consistent, accessible way to assess and support prenatal attachment, despite overwhelming evidence that this early connection matters.
Where many postnatal models begin too late, and traditional antenatal assessments are based on checklists or self-report scales, Pregnancy BASICS takes a different approach. It focuses on:
- Simple, respectful, trimester based questions that allow parents to reflect on their own experience of pregnancy and their parent-fetal relationship
- What the practitioner sees, hears, and senses in the parent–fetal relationship
- Identifying when families may need support, and enabling early help or referrals
The tool aligns closely with the Start for Life and Family Hub agenda, supporting better integration between health and early years services, and putting the developing parent–child relationship at the heart of perinatal care.
Pregnancy BASICS is structured into four easy-to-use components, which can be used flexibly during routine appointments or targeted interventions:
Open-ended questions for each trimester, such as:
These prompts are designed to invite reflection, explore emotions, and spark parent-led dialogue — without judgement or pressure.
There are six core areas of observation to notice signs of bonding:
These observations are informal, non-intrusive, and based on authentic interactions — a parent cradling their bump, using a nickname, adjusting to baby’s movements, or speaking warmly about the baby.
A simple -1 to 2 scale that helps practitioners summarise bonding across the six observation areas.
This can be used for supervision, reflective practice, or service evaluation, but it’s entirely optional.
If signs of bonding are low or inconsistent, the tool provides clear next steps and signposting for support:
Pregnancy BASICS has been designed to work across a range of services and practitioners, including:
Whether you have 10 minutes or 60, the tool offers a way to gently observe, open up conversation, and build a clearer picture of the parent–fetal relationship.
Unlike many tools designed to support postnatal assessments, this tool recognises that bonding begins long before birth and is specifically designed for use during this critical period before birth.
There are no required scoring sheets (you can use the scale if you choose), no medicalised language, and no intrusive questions. It's designed for all - mums, dads & partners.
Whether you’re in a Family Hub, a maternity team, health visiting or working in Early Help, Pregnancy BASICS is flexible, relational and ready to use. It has been thoughtfully designed with cultural sensitivity and neurodiversity in mind, ensuring it is inclusive, adaptable, and suitable for use across all communities.
